Testing Forms for Faculty

Exams must be received at least one hour prior to closing for processing to begin. Exams submitted after 7:00 pm Monday through Thursday or after 3:30 pm Friday and Saturday will be dated as being received the following business day.

Despite expanded hours, we will maintain the 48 business hour processing time.  Sundays are not counted in determining when tests will be available.

Monday – Thursday
 cut off time to make a test available in 2 days is 7:00pm

Friday and Saturday
cut off time to make a test available in 2 days is 3:30pm

Sunday
not included in 2 day processing time = Monday submission date

There are several forms that are used in the Testing Center. Read this information carefully. See downloadable forms below (requires Adobe Acrobat Reader or MS Word).

TO ENSURE EFFICIENCY AND QUALITY OF SERVICE, SUBMIT ALL TESTS AT LEAST 48 HOURS PRIOR TO ADMINISTRATION

Distance Learning Test Administration Request Form (required for all distance learning tests)

This form is filled out by the instructor who would like to have his or her exams administered in the Testing Center. On this form the instructor provides the guidelines of how the exam is to be taken by the student. Tests will not be accepted without a completed form. If information is missing from the form, the administration of tests to your students may be delayed.

Classroom/Instructional Makeup Request Form

This form provides makeup test guidelines for students who missed classroom examinations due to documented circumstances beyond their control. (i.e. medical emergency, court appointment, etc.). Makeup exams are not administered for an entire class. Instructors are asked to limit their request to five or fewer students. One form is to be filled out per student. Makeup exam forms for Dublin and Westerville instructors are available from front desk personnel.

Blackboard Testing - Securexam Course Information Sheet

This form is filled out by an instructor who would like to have his or her exams administered in the Testing Center on Blackboard. On this form, instructors will provide the steps of how the students will access the exam. Instructors must provide one paper copy of their test should there be a system outage and/or computer updates running.

  • You will need to fill out the top portion of the form with the instructor's name, course name, number and 5-digit synonym, test, and current quarter.
  • Steps 4 & 5 will be the directions for your students to follow to access their test. This information must be provided by you.

Test Denial Form

This is given to a student when he/she is denied an exam for some reason (e.g., arrived late to the Center, had no photo identification, test was not available, deadline was passed, etc.). A copy is given to the student and a copy is mailed to the instructor stating why the student was not given the opportunity to test.
